Six Month Policy (4 Pay Plan)

15% down payment is due on the effective date of policy.

The balance will be divided equally among the remaining 3 installments and will be due every month.

Reminders for Policyholders:

• Endorsements effective during the billing period will be split between any remaining payments.
• Endorsements effective after the billing period will be due in full by the date indicated on the bill.
• If your policy is billed to a third party, a copy of the invoice will go to the named insured as well as the third party.
• All coverage questions, changes and claims should be handled through your agent/producer.

*If you choose a payment plan, a $6.00 service fee will be added to each billing. There will be a $30.00 fee for dishonored checks.
